Focus 10 Life® is a proprietary executive life insurance benefit plan that was designed with input from employers and brokers. Focus 10 Life® is underwritten by Ameritas Life Insurance Corp. and administered by FOCUS 10 LIFE, Inc. Typical clients include law firms, medical practices, municipalities, and even small, closely held companies.

Ameritas Life Insurance Corp., along with Ameritas Life Insurance Corp. of New York, join to make Ameritas Mutual Holding Company; an affiliated group of life insurers offering a wide range of insurance and financial services to businesses and individuals. Ameritas Mutual Holding Company is headquartered in Lincoln, NE.

FOCUS 10 LIFE, Inc. is the exclusive administrator of Focus 10 Life®. The company was formed specifically to administer this product and has been doing it since 2001. FOCUS 10 LIFE, Inc. handles all case design, proposals, enrollments and administration of Focus 10 Life® in its Glastonbury, CT office.

Almost. It’s called “Simplified Issue” because there is one question for eligibility:

Have you been actively at work on a full-time basis? “Actively at work” means performing your normal duties for at least 30 hours per week for the past 3 months with no absences totaling 5 consecutive days or more due to illness or accident.

In these situations, eligibility is determined on a case-by-case basis. If the employee has missed 5 or more consecutive days in the past 3 months due to illness or injury, complete an application and list the reason for absence. If the employee is out at the time applications are being completed, the earliest they could become eligible for Focus 10 Life® is when he/she has been “actively at work” for 3 months. The employee must submit a new application at that time.

Rates are based on age, gender, and tobacco/nicotine use.

No. An insured who smokes 2 or fewer cigars per month qualifies for non-tobacco/nicotine rates.

Policy/Policies can be owned by the employer, insured, a trust or other designated 3rd party with an insurable interest.

No. All members of an eligible class must participate. Premiums are paid by the employer.

Focus 10 Life® is fully portable. The policy can be continued at the same rates the employer pays. It is convertible to permanent insurance.

There are several options:

1. The employer (or each employee) could continue to pay premiums. They increase substantially in year 11, so this is likely to be less attractive than the other options.

2. The employer could choose to “restart” their plan. Each employee would complete a new application. New 10 year term policies would replace the existing policies.

3. Policies may be converted to permanent insurance, without proving insurability.
